HISTORY FROM MOTHEROn direct Question: Headache, herbal medication, sore throat, and skin infection , oliguria+, easy fatigability+, palpitations +, seizures, vomiting + already eaten food, diarrhea- cough +On examination Young male, Afebrile, Hydration fair, mild pedal edema, urethral catheter in situ (50mls)Tone reduced on the right wit power on the right 0/5 on both upper and lower limbs and 4+/5 on the left. Glasgow coma scale 15/15Central nervous system pupils reactive to light, strabismus of the left eye Slight deviation of the mouth to the leftChild was well until about 3 weeks ago when grandmother realized she had early morning facial puffiness so she thought it will resolve on its own so she didn’t send the child to any facility for management.2 weeks ago child’s facial puffiness worsened so they took child to a peripheral facility where she was treated and given some medications. The condition worsened so they then took child to St. Johns of God (Duayaw Nkwanta) hospital where child was treated seen and an impression of chronic kidney disease was made. Child’s Hemoglobin was 5.1g/dl so she was transfused but during the transfusion she had a reaction so the transfusion was stopped. So she was referred to our facility for further management.
