ABSTRACTHypertensive Disorders of Pregnancy (HDP) are the commonest medical conditions in pregnancy and a major cause of maternal and perinatal morbidity and mortality. Although, HDP has been well researched in the Ghanaian teaching hospitals, there is paucity of literature on works from the secondary and primary facilities. This study aimed to determine adverse maternal and perinatal outcomes as well as their risk factors among women with HDP in the Central Region of Ghana. An analytical cross sectional study collected data on the socio-demographic and maternal characteristics, maternal and perinatal outcomes using a pretested data extraction form from antenatal, delivery and postnatal record books of women with HDP. Crude and adjusted relative risk (RR), with 95% confidence intervals associated with adverse maternal and perinatal outcomes were compared using multivariable binomial regression, P< 0.05 is considered significant. A total of 190 women with HDP were studied. Majority (77.7%) were diagnosed as preeclampsia. Adverse maternal outcomes were MSAF-30.6%, PROM-18.9%, Hemorrhage-56.8% (antepartum 8.9% and postpartum 47.9%), Renal dysfunction-5.3%, Hepatic dysfunction-2.6% and Intracranial hemorrhage-2.6%.Adverse perinatal outcomes were stillbirth-14.7%, abortions-2.1%, low birth weight-45.9%, low APGAR score-44% and congenital abnormality-3.2%. The risk factors for adverse maternal and perinatal outcomes were; maternal age > 35 years, ANC booking > 20 weeks’ gestation, systolic BP> 140 and diastolic BP>90 at booking and delivery at<37 weeks gestation. The need for improved maternal and perinatal care services for women with HDP cannot be overemphasized.
