CLIENT’S DEVELOPMENTAL HISTORYAccording to client’s mother, client was born at Kumasi in the Ashanti Region on the 18th of October 1995 at Komfo Anokye Teaching Hospital through a Spontaneous Vaginal Delivery by a qualified midwife with no congenital abnormalities like cleft palate, extra digits or harelip. She was breastfed for six months before supplementary foods such as cerelac and lactogen was introduced and was weaned at the age of two years when she was able to eat the family food.Client was immunized against the six childhood killer diseases such as measles, tuberculosis, yellow fever and poliomyelitis.
