HISTORYDuah Charles Prosper is a 7-year-old child born to Mr Kwaku Duah and Mrs Yeboah Leticia. He was born on 2/05/2013 at the Sunyani Regional Hospital. According to mother, she is G3P3 woman who had all her deliveries through caesarean section(C/S), two females and one male of which the male child is my patient of study. FAMILY HISTORYAccording to the mother, there are no known diseases in the family and there are no known allergies in the family as well. The family’s support system is through the National Health Insurance Scheme (NHIS) and the Tigo life Insurance. Master Duah Charles Prosper was immunized against the six child killer diseases.
